The October 23 update is available on Steam

Pathologic 3 Great October Update arrived on October 23, 2026. Ice-Pick Lodge's Steam announcements and the publisher's release notice confirm the delivered update. This report covers the PC Steam release, checked October 25, including new difficulty choices, independent subtitle settings and changes to the information shown during play.

The game casts you as physician Daniil Dankovsky in a town facing a plague, with a twelve-day campaign and opportunities to revisit decisions. That premise makes changes to resources, dialogue and time-travel feedback consequential: they affect the tools through which you navigate an already released story.

Easy and Hard join the original Normal balance

Ice-Pick Lodge describes Easy as reducing systemic pressure so players can concentrate on the story, while Hard asks for more careful resource management. Normal preserves the release experience. The detailed notes identify Amalgam availability and the effects of Mania and Apathy among the parameters affected by difficulty.

For a returning player, choose the question you want the next run to answer. If keeping track of a narrative thread was the appeal, the lower-pressure option is relevant. If planning around limited supplies was the appeal, read Hard's selection-screen explanation before committing. These are different session goals, rather than a ranking of which player deserves the story.

Voice acting and subtitle language can be selected independently

The update separates subtitle language from voice-over language. Ice-Pick Lodge specifically gives Russian audio with English subtitles as a supported combination. Steam currently lists English and Russian for interface, full audio and subtitles.

That creates a practical choice for someone who wants to hear one performance while reading another language. Check both selections rather than assuming one menu controls everything. When discussing a scene with another player, identifying the text and audio combination also makes it easier to describe which presentation you encountered. Separate selection does not establish additional unlisted translations.

Rewind costs and decrees receive clearer feedback

The patch notes say the time-travel screen now displays the correct Amalgam expenditure when restarting the current day and adds a visual expenditure effect. Individual decree icons are also added. The map draws quest markers over district-exit markers, while the People screen retains its scroll position after closing a dossier.

These changes concern reading a decision before acting on it and finding information afterward. For example, checking a displayed rewind cost is a different task from deciding whether revisiting the day serves your plan. Clearer feedback can support that decision without choosing the desired outcome for you.

Shabnak encounters receive new variations

The developers say they replaced the encounter version involving two Shabnaks and added another variation. Their explanation emphasizes attention over quick reactions; the developer letter describes one new approach in terms of hide-and-seek.

That is a reason to examine the current encounter's cues before following an older walkthrough. A sequence written for the replaced version may answer a different problem. The full patch notes explicitly contain story spoilers, so readers seeking only the feature changes can use the opening highlights before deciding whether to examine the day-by-day corrections.

Creative tools arrive alongside PC optimizations

Photo Mode adds weather, lighting and sticker options. The publisher also reports work on lower-end CPU performance and Steam Deck optimization, alongside voice-over subtitles and interface improvements.

Treat a performance comparison as a separate question from the feature list: keep the device, settings and repeatable scene visible when describing a result. This coverage confirms the documented additions and corrections; we have not benchmarked them. For a return visit, select one purpose first—revisit the story, investigate the new encounter or compose a picture—then check the relevant settings instead of changing everything at once.
